Job summary

Qualifications

• Doctor of Pharmacy degree and a valid NY pharmacist license.
• ASHP-accredited PGY-1 completion; PGY-2 preferred.
• Board certification strongly encouraged.

Responsibilities

• Demonstrates ability to perform clinical, technical, service and administrative tasks.
• Develops and implements policies to improve medication safety per ASHP standards.
• Supports Pharmacy & Therapeutics committee with drug evaluations and formulary maintenance.
• Collaborates with interdisciplinary teams to optimize medication therapy and reduce duplication of services.
• Attends rounds and documents recommended interventions and approvals.
• Engages in patient-centered medication therapy management, including reconciliation, discharge services and counseling.
• Maintains patient confidentiality and coordinates pharmaceutical care across department staff.
• Provides education to patients and caregivers about disease states and medications.
• Precepts residents and students; designs and implements staff trainings.
• Supervises pharmacy staff and participates in performance evaluations.
