Qualifications

• Doctor of Pharmacy Degree required.
• Current NYS Pharmacist license and registration.
• ASHP-Accredited PGY-1 required; PGY-2 residency preferred.
• Board certification (BCPS/BCCCP) encouraged.
• Hospital pharmacy and/or clinical experience; management experience preferred.

Responsabilités

• Participate in daily multidisciplinary internal medicine hospitalist rounds.
• Provide accurate and timely medication information to healthcare staff and patients.
• Provide clinical and operational pharmacy support to pharmacy and medicine teams.
• Develop and implement clinical pharmacy programs and services with leadership input.
• Serve on committees and participate in antimicrobial stewardship and transitions of care.
• Identify drug utilization reviews and promote rational drug therapy actions.
• Support quality and cost-saving initiatives and formulary management.
• Precept pharmacy students and residents; support workflow optimization.
• Ensure regulatory compliance and documentation; on-call coverage as needed.
