Job summary

Fairfield Senior Living & Rehabilitation LLC is seeking an Infection Preventionist (IP) who is a licensed RN or LPN to lead our Infection Prevention and Control Program. You will oversee surveillance, education, and compliance with CMS, CDC, OSHA, and IDPH guidelines to maintain resident safety.

The role offers on-site responsibilities with shift-based incentives, a $10,000 sign-on bonus, and potential on-call differentials to support patient care and regulatory readiness.

Qualifications

• Active Illinois RN or LPN license required.
• Infection prevention training or CMS-compliant certification.
• Experience in long-term care or infection prevention preferred.

Responsibilities

• Develop, implement, and oversee the facility's Infection Prevention and Control Program per CMS/CDC/IDPH guidelines.
• Conduct infection surveillance, identify trends, and lead improvement opportunities.
• Monitor hand hygiene, environmental cleaning, and PPE use; train staff on infection prevention.
