Job summary

Oak Glen Post Acute is seeking a dedicated Infection Preventionist to lead our infection prevention and control program in Cherry Valley, CA. You will ensure compliance with federal, state, and local regulations while promoting resident and staff safety.

Responsibilities include surveillance, outbreak investigations, staff training on hand hygiene, PPE, and isolation, and collaboration with nursing leadership to reduce infections.

Qualifications

• Current California RN or LVN license preferred.
• Infection Preventionist experience in a skilled nursing facility or healthcare setting preferred.
• Strong knowledge of CDC, CMS, and California Department of Public Health regulations.
• Certification in Infection Prevention and Control (CIC) preferred or willingness to obtain.
• Excellent organizational, communication, and leadership skills.
• Ability to analyze data, identify trends, and implement effective interventions.

Responsibilities

• Develop, implement, and maintain the facility's Infection Prevention and Control Program.
• Conduct surveillance, tracking, trending, and reporting of infections and communicable diseases.
• Monitor and investigate infection outbreaks and implement corrective actions.
• Ensure compliance with CDC, CMS, OSHA, and state infection control regulations.
• Collaborate with nursing leadership and interdisciplinary teams to reduce healthcare-associated infections.
• Educate and train staff on infection prevention practices, hand hygiene, PPE usage, and isolation precautions.
• Maintain infection control documentation and prepare reports for Quality Assurance and Performance Improvement (QAPI) meetings.
• Conduct audits and provide ongoing monitoring of infection prevention practices throughout the facility.
• Assist with emergency preparedness and outbreak response planning.
• Serve as a resource for residents, families, and staff regarding infection prevention initiatives.
