Job summary

Mount Sinai Health System seeks an experienced Clinical Program Coordinator (Infection Control) to lead complex care coordination across departments, develop and revise care pathways, and implement strategies to improve patient outcomes. The role interfaces with Nursing, Medical leadership, and Quality leadership to drive program development and educational initiatives.

The position requires a Master’s degree (microbiology, nursing or public health) and NYS nursing licensure preferred, with CIC

Qualifications

  • Master's degree in microbiology, nursing or public health.
  • NYS license in Nursing preferred.
  • CIC certification within three years of hire.
  • 3 years experience in specialty clinical field.

Responsibilities

  • Coordinates complex, multi-factorial clinical care within a designated specialty, in conjunction with Nursing, Social Work, Care Management and other related disciplines and leadership.
  • Establishes and implements short and long term goals, policies and procedures to ensure high-quality nursing care.
  • Organizes and leads around-the-clock patient care coordination.
  • Provides consultation and education to interdisciplinary team members.
  • Promotes interdisciplinary approach in patient care delivery.
  • Serves as a patient advocate.
  • Participates in quality improvement and compliance activities.
