Qualifications
Minimum of Kansas licensure or certification in Physical Therapy.
Clean background check.
Maintain 1st Aid, CPR, HIPAA, BBP, Abuse/Neglect/Ethics, and OSHA certifications.
Valid Kansas Driver's License with a good driving record.
Family Service Coordinator certificate (obtained after hire).
Responsibilities
Serve on a transdisciplinary team for families enrolled in the Infant Toddler Program.
Provide comprehensive and integrated support as outlined in the Part C Program Manual for Infants and Toddlers in Kansas.
Provide coaching and training to other team members and families.
