Job summary

Bon Secours Mercy Health is hiring a Community Health Worker for the Lorain market to support direct outreach and engagement with the community. The role focuses on improving health outcomes, access to services, and coordinating with health and social services in person at Mercy Lorain Hospital.

The CHW will build trust with community members, identify barriers, conduct home visits or events, and maintain records for program evaluation. Spanish language skills are strongly preferred.

Qualifications

• High School Diploma or equivalent required.
• CHW Certification preferred; must be obtained within 12 months of hire.
• 1 year of community outreach or healthcare experience preferred.
• Bilingual Spanish strongly preferred.

Responsibilities

• Builds trusting relationships with individuals and communities to mediate between individuals and health/social service systems.
• Evaluates needs and barriers (housing, food insecurity, transportation) and connects to resources.
• May conduct outreach activities, including home visits and community events.
• Maintains accurate records to support advocacy and program evaluation.
• May perform health education or basic health screenings (blood pressure, glucose) under supervision.
• Provides support to ongoing community health programs.
