Job summary

Bon Secours Mercy Health seeks a Community Health Worker for the Tiffin Hospital market to provide direct support and outreach to improve community health outcomes. The role serves as a liaison between community members and health/social services, delivering culturally appropriate health education and addressing barriers to care.

Responsibilities include building trust in communities, conducting outreach, maintaining records, and assisting with health screenings.

Qualifications

• High School Diploma or equivalent required.
• Bachelor's degree in Social Work, Public Health, Psychology, or related field preferred.
• Community Health Worker certification (preferred; required within 12 months of hire) with valid driver's license.
• 1 year of recent experience in a community outreach or healthcare setting preferred.

Responsibilities

• Builds trusting relationships with individuals and communities to mediate between individuals and health/social service systems.
• Evaluates needs and challenges (health, social, educational) and connects to resources.
• May conduct outreach activities including home visits and community events.
• Maintains accurate records to support advocacy and program evaluation.
• May perform or assist with basic health screenings under supervision.
