Job summary

Join the Cleveland Clinic Health System as a Community Health Worker in Lorain County. You will connect diverse, underserved populations to health and social services, working closely with medical providers to improve patient care and outcomes.

You will visit patients in the community and their homes, build trusting relationships, educate on health resources, and support navigation of care plans and programs. This role runs Monday–Friday 8:30 a.m. – 5:00 p.m., with occasional events.

Qualifications

High school diploma or GED.
Valid driver's license.
CHW certification and Board of Nursing CHW certification.
Ability to build trusting relationships.
Strong information gathering and time management.

Responsibilities

Establish trusting relationships with patients and families; provide support and encouragement.
Coach patients in managing chronic conditions and self-care.
Explain program purposes and services to patients and communities.
Help patients understand care plans and instructions.
Visit patients at home as needed to assess conditions and living situation.
Document activities and plans per policies.
Advocate for patients and coordinate with community agencies.
Assist with scheduling and applications for programs patient may be eligible for.
Assist accessing health services and navigating the medical home.
Coordinate communication between providers.
