Job summary

Responsibilities

• Establish trusting relationships with patients and families while providing support and feedback.
• Coach patients in managing chronic health conditions and self-care.
• Explain program purposes and services to the user population.
• Assist patients in understanding care plans and instructions.
• Visit patients in their homes as needed and assess living conditions.
