Job summary

Default Brand is seeking a Consumer Services Coordinator (Case Management) to coordinate services for individuals with developmental disabilities in the Riverside Adult Central area. Under the supervision of the Program Manager, you will develop IPPs, track progress, provide advocacy, and prepare required documentation.

This role requires travel to Riverside communities; mileage reimbursement is provided. The position offers a generous benefits package and an hourly range of $26.3130 to $40.8201.

Qualifications

• Bachelor’s degree from an accredited college or university and one year of experience, including case management, in intellectual/developmental disabilities, social work, special education, early childhood development, or a related field.
• OR Master’s degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or a related field from an accredited college or university.
• Bilingual preferred but not required.
• Ability to establish and maintain effective working relationships and good attendance/punctuality.

Responsibilities

• Coordinate case management services and develop IPP progress reports and annual reviews.
• Attend relevant meetings (IEP, ITP, IHC) and prepare paperwork.
• Monitor IPP progress and implement IPP addendums as needed.
• Complete appropriate documentation and ensure compliance with Medicaid Waiver standards.
