Job summary

Coalition for Homelessness Intervention and Prevention (CHIP) Indianapolis seeks a dedicated Housing Navigator to connect Streets to Home participants with community resources and housing options. You will provide client-facing housing navigation in coordination with the Streets to Home Director and partner agencies.

The role involves extensive outreach, data capture in HMIS, documentation, and collaboration within a multidisciplinary team to support housing stability.

Qualifications

• Associate’s degree or higher preferred in related field (social work, sociology, human services, psychology).
• 2 years’ experience navigating homelessness and/or social service systems.
• Equivalent combination of education, experience, and communication skills accepted.
• Individuals with lived homelessness experience are encouraged to apply.

Responsibilities

• Engage unsheltered individuals, assess needs, provide resources and services, including crisis management.
• Inform clients of housing options and navigate case management and housing systems.
• Locate clients with shelters and HMIS lists and obtain vital documents for housing applications.
• Administer Coordinated Entry System assessments and document accordingly.
• Transport clients using personal vehicle and arrange rides to services and appointments.
• Assist in finding housing, address barriers, show units, and attend lease signings.
• Complete housing paperwork and liaise with program, client, case manager, and landlords.
