Job summary

Aspire Indiana Health is seeking a Community Health Worker - Housing in Indianapolis. This role supports vulnerable clients with housing plans, advocacy, and community referrals to keep individuals housed and connected to essential services.

The CHW will perform home visits, help navigate housing resources, and coordinate with primary care, behavioral health, SNAP, transportation, and employment services to improve overall well-being.

Qualifications

• High School Diploma or equivalent required.
• Bachelor’s Degree in social work, psychology, counseling or related field from a college/university accredited by the US Department of Education preferred.

Responsibilities

• Engage clients in housing planning and advocacy.
• Visit clients in their home once housed and develop skills to remain housed.
• Provide ongoing assessment and refer to housing, primary care, behavioral health, SNAP, transportation, employment, etc.

Education
