Job summary

Upperline Health, Inc. is seeking a Medical Director for the Indiana market to advance clinical excellence, strengthen provider engagement and lead clinical value-based care across the market. Primary home clinic: Indianapolis.

The role blends patient care, clinical leadership, and VBC program management, partnering with the central interdisciplinary team to drive high-quality, coordinated care and meaningful improvements in outcomes for our patients across Indiana.

Qualifications

• MD or DO required.
• Board-certified or eligible in Internal Medicine, Family Medicine, Preventive Medicine, or Geriatrics.

Responsibilities

• Lead, coach and care for patients in a team-based model.
• Advance clinical excellence and lead clinical value-based care across the Indiana market.
• Collaborate with central interdisciplinary team, population health and operations teams.
