Job summary

Upperline Health Inc. seeks a Medical Director for the Indiana market to advance clinical excellence, strengthen provider engagement and lead clinical value-based care across the market.

This role combines patient care, clinical leadership, and VBC program management, partnering with central teams. The primary home clinic will be in Indianapolis, with responsibilities across clinics and home-based care.

Qualifications

• MD/DO required and board-certified or eligible in relevant specialties.
• Active Indiana medical license.
• Approximately 2 years outpatient experience with complex patients.
• Experience coaching or mentoring other clinicians.
• Willing to reside in and serve the Indiana market, with ability to provide direct care.

Responsibilities

• Lead and coach care teams in a value-based care model.
• Advance clinical excellence and engage providers.
• Partner with central interdisciplinary and operations teams.
• Manage the VBC program across the Indiana market.
• Primary home clinic in Indianapolis.
