Job summary

FreedomCare is seeking a Care Manager in Indiana to support patients and families in their homes. The role focuses on patient assessments, caregiver training, and ongoing supervision to ensure high-quality home care across the state.

You will conduct orientations, assess patient needs, and ensure caregiver readiness while collaborating with the care team. Travel within Indiana is required with mileage reimbursement provided.

Qualifications

• 2+ years of experience working with frail or elderly disabled adults and caregivers.
• Field experience in a medical or case management environment preferred.
• Salesforce experience is a plus.
• Hands-on experience in home care preferred.
• Working knowledge of ADL/IADL assessments.
• Valid driver’s license with an insured vehicle for travel within the state.

Responsibilities

• Evaluate the home care needs of the client.
• Assess the environment.
• Assess caregiver fitness to provide care.
• Conduct home orientations and sign paperwork.
• Perform initial assessments during orientations.
• Conduct periodic supervisory visits for on-care cases.
• Train caregivers and patients as required.
• Coordinate with care manager and staff.
• Engage in interdisciplinary meetings with care teams.
• Communicate case impressions to staff and stakeholders.
• Suggest process improvements and training for caregivers.
• Travel within Indiana with mileage/expenses reimbursed.
