Job summary

FreedomCare LLC in Indiana is seeking a Care Manager for our field-based team. Travel to patient homes in the Muncie area; you will orient, assess, and train caregivers to ensure high-quality home care.

The ideal candidate has 2+ years of experience with frail or elderly adults, field medical/case management experience, and familiarity with Salesforce or MS Office. A valid driver's license and insured vehicle are required.

Qualifications

• 2+ years of experience with frail or elderly adults and caregivers.
• Field experience in medical or case management preferred.
• Experience with data input and case note entry; Salesforce a plus.
• Hands-on home care experience preferred.
• Knowledge of ADL/IADL assessments.
• Valid driver’s license with insured vehicle for travel in-state.
• Excellent verbal and written communication, organizational and time management skills.
• Strong critical thinking and problem-solving skills.
• Proficient in Microsoft Word, Excel, Outlook.
• Team-player mentality.

Responsibilities

• Evaluate the home care needs of the client.
• Assess the environment and caregiver fitness.
• Conduct home orientations involving signing of paperwork.
• Periodic supervisory visits for on-care cases.
• Train caregivers and patients as required.
• Collaborate with care managers, caregivers and patients.
• Interdisciplinary meetings and communicate case impressions.
