FreedomCare is seeking a Care Manager to support patients at home in Indiana, evaluating needs, orienting families, and training caregivers to ensure high-quality care. This field-based role requires travel across the South Bend area and neighboring counties, with mileage reimbursement provided.

You will collaborate with care teams, perform orientations and initial assessments, and monitor caregiver performance to improve patient outcomes.

Formación

• 2+ years of experience working with frail or elderly adults
• Field experience in medical or case management preferred
• Salesforce experience is a plus
• Hands-on experience in home care
• Knowledge of ADL/IADL assessments
• Strong communication and organization skills

Responsabilidades

• Evaluate the home care needs of the client
• Assess the environment
• Train caregivers on care delivery
• Home orientations involving paperwork
• Initial assessments during orientations
• Periodic supervisory visits for on-care cases
• Interdisciplinary meetings with caregivers and patients
• Travel within Indiana with mileage reimbursement
