Job summary

FeldCare Connects seeks a self-motivated Occupational Therapist to deliver premier care in the Temecula area. This position is an Independent Contractor role serving adults of all ages in home health. You will assess patient needs, plan treatments, and guide the care team to improve daily functioning and safety.

Responsibilities include implementing treatment plans, documenting progress, and educating patients and families. A CA OT license is required; bilingual candidates are a plus.

Qualifications

Ability and enthusiasm in working with all ages.
Occupational Therapist license and registration by the state.
Completion of an accredited Occupational Therapist program.
Bilingual a plus!

Responsibilities

Assess and evaluate patient care needs related to functioning status, activities of daily living, fine motor coordination, and home assessments.
Determine a treatment plan based on assessment and evaluation, perform interventions, and evaluate progress.
Lead patients in therapeutic, creative, and self-care activities to improve functioning and safety.
Observe, record, and report patient responses and changes to condition; instruct patients, families, caregivers, and healthcare team.
