Job summary

Snap Health in Irving, TX seeks an Independent Contractor Occupational Therapist to provide in-home or facility-based OT services. You will evaluate patients, develop plans of care, and collaborate with physicians and care teams to restore function and relieve pain.

Qualifications

• Must be currently licensed to practice as an Occupational Therapist in the STATE OF TEXAS
• Must possess and maintain valid CPR certification.
• Valid driver’s license, auto insurance and reliable transportation

Responsibilities

• Evaluates the patient’s functional status and occupational therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by the state board of occupational therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Treats patients to relieve pain, restore function, and / or maintain maximum functioning using acceptable standards of practice.
• Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
• Visits patients according to Plan of Care, completes progress notes timely.
• Provides supervision of COTAs/OTAs
• Participates in discharge planning for patient.
