Job summary

Snap Health in Garland, TX is seeking an Occupational Therapist or COTA for a contract role in Home Health. You will assess patients, develop and implement plans of care, and collaborate with the care team to achieve optimal outcomes.

Qualified applicants must hold Texas licensure, CPR, and reliable transportation. Experience in home health is a plus. You will supervise COTAs, contribute to discharge planning, and pick up shifts that fit your schedule.

Qualifications

• Must be licensed to practice as an Occupational Therapist or COTA in Texas.
• CPR certification required.
• Driver’s license and reliable transportation.
• Home health experience is a plus.
• Ability to work independently and in a team.

Responsibilities

• Evaluates patient functional status and OT needs; assists physician with plan of care.
• Provides OT services within plan of care and scope of practice.
• Treats patients to relieve pain, restore function, and maximize functioning.
• Instructs patient and family about plan of care and progress.
• Visits patients according to Plan of Care and completes progress notes timely.
• Supervises COTAs/OTAs.
• Participates in discharge planning for patient.
