Job summary

TheraCare in TX is seeking a licensed Occupational Therapist (OT) for full-time and PRN home health positions, delivering one-on-one in-home therapy to adults and seniors throughout Baytown and surrounding areas.

You will evaluate, treat, and develop plans of care, address ADLs/IADLs, and collaborate with physicians and care teams while maintaining accurate EMR documentation. Flexibility, autonomy, and travel within the Baytown region are expected.

Qualifications

• Doctorate in Occupational Therapy from an accredited program.
• Active OT license in Texas or eligible for Compact licensure.
• Home Health experience (1 year required).
• HomeCareHomeBase (HCHB) experience preferred.
• Current CPR certification.
• Strong communication, clinical judgment, and time-management skills.
• Knowledge of home health regulations and Medicare guidelines preferred.
• Valid driver's license, reliable transportation, and current auto insurance.

Responsibilities

• Perform home health occupational therapy evaluations and re-evaluations for adult and geriatric patients
• Develop and update individualized Plans of Care (POC)
• Treat patients with orthopedic, neurological, cardiac, post-surgical, or other medical conditions
• Address ADLs, IADLs, coordination, strength, balance, and functional mobility
• Apply manual therapy techniques and non-complex modalities, including heat, cold, electrical stimulation, and other physician-ordered treatments
• Implement interventions for fall prevention, home safety, and environmental modifications
• Train patients and caregivers in adaptive equipment and durable medical equipment (DME)
• Fabricate splints and recommend assistive devices as appropriate
• Complete accurate and timely documentation in the EMR
• Communicate with physicians and interdisciplinary care teams
• Supervise COTAs and home health aides, as applicable
• Follow all home health regulations, infection control, and safety standards
