Job summary

Nurturing Home Healthcare is seeking a compassionate Occupational Therapist to join our Home Health team on a PRN basis in Houston, TX. The OT will evaluate patients in their homes, develop individualized treatment plans, and implement interventions to enhance daily living activities and independence.

The ideal candidate should be patient-focused, clinically skilled, and capable of collaborating with physicians, nurses, caregivers, and other professionals to ensure high-quality patient care.

Qualifications

• Active Texas OT license.
• Graduate of accredited OT program.
• Minimum 1 year OT experience.
• CPR certification or ability to obtain.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive OT evaluations in patients' homes.
• Develop individualized treatment plans based on physician orders.
• Assess ADLs and IADLs to guide interventions.
• Provide therapies to improve strength, mobility, cognition, and independence.
• Educate patients and caregivers on safe practices and home exercise programs.
• Coordinate care with physicians and other health professionals.
• Document patient progress and treatment outcomes per agency policy.
