Walmart is seeking an Independent Optometrist leasing space inside Sam's Club locations. You will lead eye and vision care while safeguarding patient well‑being, with autonomy to set days of practice and hours, control your schedule, and determine your fee schedule.

As a lease holder you retain 100% of exam and professional fees and may opt into equipment, instrumentation, and support services where allowed by state regulations.

Qualifications

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD or DO) from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9.
