Sam's Club Independent Optometrist leasing space inside Walmart stores will take a leading role in patient care with respect to eye and vision care as well as general health and wellbeing. As an independent lease holder, you control your days of practice, hours of operation, patient scheduling, and fee schedules.

You can keep 100% of exam/professional fees and may opt into advanced equipment support and administrative assistance if state permits. Primary location: Apple Valley, MN.

Qualifications

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist MD or DO from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate/Professional Liability Insurance Certificate.
• ME/VA license agreements are with Gumberg Asset Management Company.
