Qualifications

• OD from an accredited optometry school.
• Valid state optometry license.
• MD or DO in Ophthalmology from an accredited school (optional).
• Valid state ophthalmology license (if applicable).
• General Liability/Professional Liability Insurance certificate.
• ME/VA license agreements with Gumberg Asset Management Company.

Responsibilities

• Launch optometry services in Sam's Club and lead patient care.
• Establish your days of practice and hours of operation.
• Control your patient schedule and fee schedule.
• Keep 100% of exam/professional fees.
• Opt into advanced equipment and support services where state allows.
