Qualifications

• OD from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD/DO) license from an accredited school; valid state ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W-9.
• ME & VA license agreements are with Gumberg Asset Management Company.

Responsibilities

• Launch your services in Walmart stores.
• Lead patient care in eye and vision care as well as general health and well-being.
• Establish your days of practice, hours, patient schedule, and fee schedule; keep 100% of exam/professional fees.
