Job summary

Dorset County Hospital NHS Foundation Trust is seeking a clinically autonomous practitioner to provide direct access to a Nurse Practitioner, either in the Practice, by telephone, or via domiciliary visits, for the general practice population.

You will act as an independent prescriber, order tests, interpret results, and work with multidisciplinary teams to deliver integrated care. You must maintain professional registration and engage in CPD and annual appraisal.

Responsibilities

• Provide direct access to a Nurse Practitioner in the Practice, over the telephone and via domiciliary visits, for the general practice population.
• Make professionally autonomous decisions and be accountable for those decisions.
• Provide a first point of contact for patients with undifferentiated problems, using history taking, examination and clinical decision‑making to establish diagnosis and management plan.
• Instigate diagnostic tests or investigations and interpret results.
• Act as an independent prescriber within current legislation to prescribe medications.
• Provide safe, evidence‑based, cost‑effective patient care and develop personalised care plans.
• Collaborate with MDTs and external organisations to deliver integrated pathways of care and meet quality targets (e.g., QOF).
• Record accurate consultation data per latest NMC guidance and participate in CPD and appraisal.
