Job summary

You will assess, diagnose, and manage patients, provide treatment and advice, support long-term condition management, and ensure thorough record-keeping in line with clinical guidelines.

Qualifications

• Qualified Nurse Practitioner with an active, unrestricted license.
• Experience delivering patient-centred care across acute and chronic conditions.
• Ability to work autonomously and within a multidisciplinary team.

Responsibilities

• Assess, diagnose and manage patients with minor illness and acute conditions within your scope of practice.
• Provide clinical advice, treatment and safety-netting to ensure timely, appropriate care.
• Support ongoing medical and nursing needs, and contribute to long-term condition management.
