Job summary

Your World Recruitment Group is seeking an experienced Pharmacist Prescriber to join a primary care team operating remotely. You will deliver specialist clinical pharmacy services and optimise medicines management for a patient population.

The role requires independent prescribing qualifications, strong clinical knowledge, and collaboration with GPs, nurses, and other healthcare professionals. CPPE pathway completion is essential.

Qualifications

• Master's degree in Pharmacy and GPhC registration required.
• CPPE pathway completed or enrolled is essential.
• Postgraduate Clinical Pharmacy qualification or RPS credentialing is preferred.

Responsibilities

• Independently assess patients and prescribe within competence and specialist area.
• Lead medicines optimisation initiatives to improve outcomes and reduce harm.
• Conduct structured medication reviews for complex patients.
• Manage medicines reconciliation and transfers of care.
• Support chronic disease management through clinical reviews with GPs, nurses, and others.
• Identify and resolve prescribing issues and drug interactions.
• Develop prescribing guidelines aligned with national/local policies.
• Lead clinical audits and evaluate prescribing practices for improvement.
• Provide clinical education and training to colleagues.
• Contribute to medicines safety initiatives and quality improvement projects.
• Develop integrated pharmaceutical care plans for patients.
• Build relationships with community and secondary care providers.
