Job summary

Reliant Medical Group, part of the Optum family, seeks a Nurse Practitioner or Physician Assistant to join its Internal Medicine team in Milford. You will independently manage a primary care panel, provide preventive, acute, and chronic care, and collaborate with a multidisciplinary team using EPIC and DAX AI ambient scribe tech to ease documentation.

Qualifications

• Board Certified NP or PA with MA licensure and active DEA.
• Minimum of 3 years of panel-managed APC primary care experience.
• Experience with EPIC and AI-assisted documentation is a plus.

Responsibilities

• Independently manage a panel of primary care patients.
• Provide preventive, acute, and chronic disease management.
• Collaborate with RNs, pharmacists, behavioral health, specialists, and population health teams.
• Support shared decision-making and whole-person care.
• Participate in team-based primary care call structure.
• Utilize DAX AI ambient scribe and EPIC tools to reduce documentation burden.
