Job summary

Reliant Medical Group in Milford, MA, part of the Optum family, seeks an experienced Nurse Practitioner or Physician Assistant to join its Internal Medicine team. You will independently manage a primary care panel in a collaborative, value-based care setting supported by advanced technology.

Join a multidisciplinary care team, provide preventive, acute, and chronic disease management, and contribute to shared decision‑making and whole-person care using EPIC tools.

Qualifications

• Board Certified NP or PA
• Must have at least 3 years of paneled APC experience
• Unlimited Licensure in MA
• Active DEA License

Responsibilities

• Independently manage a panel of primary care patients
• Provide comprehensive preventive, acute, and chronic disease management
• Collaborate with RNs, pharmacists, behavioral health, specialists, and population health teams
• Support shared decision‑making and whole-person care
• Participate in a team-based primary care call structure
• Utilize EPIC tools and DAX AI ambient scribe to reduce documentation burden
• Join a high-performing, clinician-led organization focused on quality outcomes and clinician well-being
