Job summary

Kona Medical Consulting is seeking a Home Health Field LVN to provide skilled nursing care to patients in their homes across the Dallas–Fort Worth Metroplex. You will work under an RN Case Manager, performing wound care, meds administration, and vital signs while supporting the care plan with the clinical team.

The ideal candidate has Texas LVN licensure and at least 1 year of nursing experience, preferably in home health.

Qualifications

• Current Texas LVN license.
• 1+ year nursing experience (Home Health preferred).
• Strong assessment, communication, and clinical skills.
• Reliable transportation, valid driver's license, and auto insurance.
• Familiarity with EMR/mobile charting.

Responsibilities

• Provide skilled nursing care under RN supervision.
• Perform wound care, medication administration, vitals, and disease process teaching.
• Assist with care plan implementation and updates.
• Conduct patient assessments and report changes promptly.
• Collaborate with physicians and team to ensure quality care.
• Maintain accurate EMR documentation.
• Travel to patient homes within an assigned service area.
