Kona Medical Consulting and The Home Health Team is seeking a dedicated Home Health LVN to deliver skilled nursing in patients’ homes across the Dallas–Fort Worth Metroplex. You’ll work under an RN Case Manager, providing wound care, meds administration, vital signs, and patient education with a supportive clinical team.

The ideal candidate is dependable, clinically confident, and comfortable working independently while maintaining thorough documentation in the EMR.

Qualifications

• Current Texas LVN license.
• 1+ year of nursing experience; home health preferred.
• Strong assessment and communication skills.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide skilled nursing care under RN supervision.
• Perform wound care, medication administration, vitals, and teaching.
• Assist with care plan updates and implementation.
• Conduct patient assessments and report changes promptly.
• Collaborate with physicians and RNs to ensure quality care.
• Maintain timely EMR documentation.

Job Overview

We are seeking a dedicated Home Health Field Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in their homes. The ideal candidate is dependable, clinically confident, and enjoys working independently while still being supported by a strong clinical team.

Responsibilities

• Provide skilled nursing care under the supervision of an RN Case Manager

• Assist with care plan implementation and updates
• Conduct patient assessments and report changes in condition promptly
• Collaborate with physicians, RNs, and other team members to ensure quality care
