Job summary

Kona Medical Consulting and The Home Health Team is seeking a Home Health Field LVN to provide skilled nursing in patients' homes across the Dallas–Fort Worth Metroplex.

The ideal candidate will be dependable, clinically confident, and capable of working independently while supported by an RN Case Manager and clinical team. Must have Texas LVN license and be comfortable traveling to patient homes. Competitive pay, mileage reimbursement, and flexible scheduling are provided.

Qualifications

• Texas LVN license required and active.
• 1+ year of nursing experience (home health preferred).
• Reliable transportation, valid driver’s license, and auto insurance.
• Familiarity with EMR/mobile charting.

Responsibilities

• Provide skilled nursing care under the supervision of an RN Case Manager
• Perform wound care, medication administration, vitals, and disease process teaching
• Assist with care plan implementation and updates
• Conduct patient assessments and report changes in condition promptly
• Collaborate with physicians, RNs, and other team members to ensure quality care
• Maintain timely, accurate documentation in the EMR
• Travel to patient homes within an assigned service area
