Kona Medical Consulting and The Home Health Team in Dallas–Fort Worth is seeking a dedicated Home Health Field LVN to provide skilled nursing care in patients' homes. The ideal candidate is dependable, clinically confident, and enjoys working independently while supported by a strong clinical team.

You will perform wound care, medication administration, vitals, and disease teaching, travel to patient homes, and collaborate with RNs and physicians.

Qualifications

• Current Texas LVN license.
• 1+ year nursing experience (home health preferred).
• Strong assessment, communication, and clinical skills.
• Reliable transportation, valid driver’s license.
• Familiarity with EMR/mobile charting.
• Compassionate, dependable, and patient-focused.

Responsibilities

• Provide skilled nursing care under supervision of an RN Case Manager.
• Perform wound care, medication administration, vitals, and disease process teaching.
• Assist with care plan implementation and updates.
• Conduct patient assessments and report changes in condition promptly.
• Collaborate with physicians, RNs, and other team members to ensure quality care.
• Maintain timely, accurate documentation in the EMR.
• Travel to patient homes within an assigned service area.
