Job summary

Kona Medical Consulting and The Home Health Team is seeking a Home Health Field Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in Dallas–Fort Worth metroplex. The ideal candidate is dependable, clinically confident, and enjoys working independently while supported by a strong clinical team.

Responsibilities include wound care, medication administration, vitals, disease process teaching, and collaborating with RNs and physicians.

Qualifications

• 1+ year of nursing experience (Home Health preferred)
• Strong assessment, communication, and clinical skills
• Familiarity with EMR/mobile charting
• Reliable transportation, valid driver’s license, and auto insurance
• Compassionate, dependable, and patient-focused

Responsibilities

• Provide skilled nursing care under the supervision of an RN Case Manager
• Perform wound care, medication administration, vitals, and disease process teaching
• Assist with care plan implementation and updates
• Conduct patient assessments and report changes in condition promptly
• Collaborate with physicians, RNs, and other team members to ensure quality care
• Maintain timely, accurate documentation in the EMR
• Travel to patient homes within an assigned service area
