The role emphasizes person-centered planning, advocacy, and collaboration with teams to promote self-determination, community engagement, and safe, supportive environments. A valid driver’s license and reliable transportation access are required.

Qualifications

• High School Diploma, GED or equivalent is required.
• Valid Driver’s License and acceptable driving record are required.
• Reliability, willingness to learn, and being open to new opportunities are essential.

Responsibilities

• Assist in developing individualized, person-centered service and support plans and play an active role in their implementation.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Assist individuals with accessing transportation and community resources as needed.
• Implement activities that promote community inclusion, relationship-building, personal growth, and the development of valued social roles.
• Maintain a safe, clean, and supportive environment that promotes health, safety, and well-being of individuals served.
• Review, complete, and maintain all required documentation in an accurate and timely manner.
