Qualifications

• High School Diploma, GED or equivalent required.
• Valid Driver’s License and acceptable driving record required.
• Reliability and willingness to learn new opportunities.

Responsibilities

• Assist in developing individualized, person-centered service and support plans and participate in implementation.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Assist individuals with transportation and community resources as needed.
• Implement activities promoting community inclusion, relationships, and growth.
• Maintain a safe, clean, and supportive environment and complete required documentation.
