Job summary

Community Bridges, Inc. seeks an Independent Living Specialist to work with SMI clients in homes or in the field to build independent living skills such as budgeting, transportation, and self-care.

The role is field‑based about 80% of the time, with after‑hours and on‑call supports as needed. Ideal candidates have a high school diploma or GED (associates preferred), 1 year of SMI experience, recovery‑oriented approach, and CPRP certification within 12 months.

Independent Living Specialist

CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

Job Summary

The F/ACT Independent Living Specialists work with SMI clients in their homes or in the field to develop independent living skills. Independent living skills include, but are not limited to, food shopping, learning bus routes, cleaning home, proper hygiene, maintaining relationships with supportive family and developing community‑based supports. This position is expected to be in the field at least 80% of the time working with clients assisting them develop skills that will assist in sustaining ongoing recovery. This position will also provide after hour, and on‑call supports.

Education and Experience Requirements
  • Highschool diploma or GED required. Associates Degree or higher preferred.
  • 1-year experience working with the SMI population with additional experience in multiple settings and levels of care, including both mental health and substance abuse preferred.
  • 1-year experience in a behavioral health position providing outreach and engagement activities preferred.
  • Minimum of six months of recovery from substance use and/or mental health
    disorders preferred.
  • Clear 39-month Motor Vehicle Record.
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
  • Certified Psychiatric Rehabilitation Practitioner (CPRP) within 12-months of employment
