Job summary

OneWell Health Care is seeking an Independent Living Skills (ILS) Trainer to provide individualized instruction and support to individuals with developmental disabilities. Training occurs in participants’ homes and community settings, following service plans and state/federal regulations.

The role emphasizes safety, confidentiality, and collaboration with supervisors, families, and the interdisciplinary team to promote independence and community integration.

Qualifications

• High School Diploma or GED required.
• Experience with individuals requiring community or residential support preferred.
• At least 1 year of experience with developmental/disabilities or related populations preferred.
• CPR and First Aid certifications required or obtainable within timeframe.
• Relevant certifications (Crisis Prevention/Intervention, etc.) preferred.

Responsibilities

• Provide individualized ILS training in home and community settings.
• Develop and implement person-centered service plans.
• Teach essential life skills: budgeting, meal prep, household management, personal hygiene, transportation, safety, communication, and social skills.
• Monitor progress and maintain professional communication with supervisors, families, and the interdisciplinary team.
• Complete documentation: assessments, progress notes, service records, incident reports, and timesheets.
• Maintain participant safety, confidentiality, and professional boundaries.
