Job summary

OneWell Health Care is seeking an Independent Living Skills (ILS) Trainer in Cross, SC to provide individualized instruction in participants' homes and community settings. You will develop person-centered service plans, teach essential life skills, and support independence and community inclusion, all while adhering to HIPAA and state regulations.

Qualifications

• High school diploma or GED is required.
• Experience in community or residential settings preferred.
• Ability to communicate effectively in English; bilingual is a plus.

Responsibilities

• Provide individualized ILS training in home and community settings by conducting assessments and teaching essential life skills.
• Develop and implement person-centered service plans and document progress in records.
• Maintain confidentiality and follow HIPAA and organizational policies; coordinate with supervisors, families, and interdisciplinary team.
• Ensure participant safety and quality of documentation; support quality improvement initiatives.
