Open Sky Community Services is seeking a Case Manager ILS to support individuals in building social skills, coordinating day supports, leisure activities, and financial management. You will develop and monitor individualized service plans and help individuals access community resources to promote independence.

The role requires a High School Diploma and a valid driver’s license; a BA in Behavioral Sciences is preferred. Alternative education or experience may be substituted.

Aufgaben

• Assist in development, implementation, monitoring, and review of individualized service plans and life goals.
• Provide support aligned with each individual’s service plan and needs to promote independence.
• Assist individuals to access and coordinate community resources including medical, behavioral health, housing, transportation, and employment.
• Facilitate opportunities for meaningful relationships, community connections, and natural supports.
• Support daily routines, scheduling, organization, and documentation as per plan.
• Maintain safe living environments and assist with transportation and appointments as needed.
• Communicate changes in needs to the Program Coordinator and support team; participate in team meetings.
• Maintain accurate documentation, service records, and billing information; ensure policy compliance.
