Trinity Services, Inc. is seeking an Independent Living Coach in Stabilization Home Services under the Team Leader to provide supervision, guidance, and assistance to people served. The program aims to foster independence and positive personal outcomes in a dignified, respectful manner.

The role requires 1 year of DSP experience in IDD or mental health. The position involves implementing individual service plans, documentation, and providing transportation and daily living supports.

Qualifications

• 1 year of DSP experience in IDD or mental health field.
• 1,000 hours of direct support work.
• Letter of references from a person supported and a supervisor.
• High school diploma or GED required; Bachelor's preferred.
• Valid driver’s license.
• CPR, First Aid, and Safety Care current.
• Pass reading comprehension test.
• Pass NADD DSP certification within 60 days of hire.

• Advocates for the rights of the persons served.
• Interacts with the people served in a supportive and encouraging manner.
• Provides supports for a safe and healthy living environment.
• Provides opportunities to learn and gain independence.
• Assists with daily living activities and community involvement.
• Ensures proper medication administration and medical attention.
• Provides transportation to jobs, appointments, and tasks, and maintains the vehicle.
