Horizons, Inc is hiring a Support Coach (Direct Support Professional) to empower individuals with developmental disabilities in a Supported Living setting. You will assist with daily tasks, meal prep, and personal goals, while ensuring safety and dignity.

Qualifications

• Minimum 2 years of relevant experience.
• CPR/First Aid training preferred or required.
• High school diploma or equivalent and valid CT driver's license.

Responsibilities

• Assist individuals with daily living tasks with respect and encouragement.
• Help with meal prep and maintaining a welcoming home environment.
• Support individuals toward ISP goals and personal interests.
• Administer medications per physician orders and DDS guidelines.
• Communicate clearly and document work; participate in training.
• Ensure safety and respond compassionately during challenging moments.
