Job summary

Area Agency on Aging & Disabilities of Southwest Washington seeks a qualified professional to assess clients using standardized tools and develop person-centered care plans. The role authorizes in-home and community services, supports caregivers, and promotes inclusive access to programs across several counties in SW WA.

The position emphasizes travel to clients’ homes, coordination with providers, and maintaining client records, with a focus on helping adults with disabilities and older persons

Qualifications

• Master’s Degree in behavioral or health science and 1 year paid on-the-job social service experience.
• Bachelor’s Degree in behavioral or health sciences with 2 years paid on-the-job social service experience.
• Bachelor’s Degree and 4 years paid on-the-job social service experience.
• Possession of a valid driver’s license and minimum state-required vehicle insurance.

Responsibilities

• Assess clients using standardized assessment tools.
• Develop a person-centered plan of care with identified outcomes.
• Authorize in-home and community services per client’s plan.
• Promote diversity, equity, inclusion and access to programs.
