Job summary

The SW Washington Council of Governments on Aging is seeking a dedicated case management professional to assist adults with disabilities and older adults. You will assess needs, authorize in-home and community services, and develop service plans to maximize independent living and timely returns home after hospital stays.

Responsibilities include comprehensive assessments, care planning, and coordinating with a network of providers while maintaining client records.

Qualifications

• Master’s degree in behavioral or health science; social service experience favored.
• Bachelor’s degree plus paid on-the-job social service experience required or equivalent.
• Commitment to serving adults with disabilities and older adults.

Responsibilities

• Perform comprehensive functional assessments and reassessments for clients.
• Develop an appropriate plan of care to meet identified outcomes.
• Implement short- and long-term service plans with in-home and community services.
• Coordinate with service providers and resolve issues in service delivery.
• Maintain client records and documentation.
• Coordinate services with mental health and disability service organizations.
