Job summary

Perfect Care Inc. seeks a Community Support Professional to help clients achieve independence by providing person-centered support across housing, healthcare, employment, and personal development.

You will work to integrate clients into the community through tailored services and local resource connections. Responsibilities include coordinating referrals, maintaining service plans, documenting encounters within 24 hours, and collaborating with families and providers to support ongoing progress

Qualifications

• Minimum of 2 years of experience in homecare or community-based human services.
• Excellent communication, documentation, and organizational skills.
• Valid driver’s license and reliable transportation preferred.

Responsibilities

• Establish rapport with clients and uphold professional boundaries.
• Assist clients to access housing, healthcare, and social resources.
• Maintain regular contact per service plan.
• Coordinate referrals for medical care, entitlements, transportation, and education.
• Develop and monitor person-centered care plans.
• Document encounter notes within 24 hours.
• Submit monthly summary reports by the 21st.
• Support client independence and community integration.
