Job summary

ConnectedHealth is seeking a Full-Time Speech Language Pathologist to deliver in-home services across Spokane, WA. You will evaluate and treat adults with speech, language, cognitive, and swallowing disorders, developing individualized plans and educating families and caregivers.

Travel to patient homes is required; strong assessment, documentation, and communication skills are essential. Idaho licensure or eligibility, plus CCC/ASHA credentials, are highly valued for this role.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current Idaho SLP license or eligibility for Idaho licensure.
• CCC from ASHA (preferred/required per licensing).
• Current ASHA certification (preferred).
• Current CPR certification (preferred).
• Valid driver's license and reliable transportation.
• Ability to travel to patient homes throughout assigned service area.
• Previous home health speech therapy experience (preferred).
• Strong clinical assessment, documentation, and organizational skills.

Responsibilities

• Deliver speech-language pathology services in patients' homes.
• Evaluate patients with speech, language, communication, cognitive, and swallowing disorders.
• Develop individualized treatment plans based on diagnoses and functional goals.
• Provide therapeutic interventions to improve communication, cognition, and swallowing function.
• Assess and treat patients with neurological, medical, and age-related conditions.
• Educate patients, family members, and caregivers about treatment plans and strategies.
• Monitor progress and modify treatment plans as necessary.
• Complete accurate and timely clinical documentation per home health standards.
• Collaborate with physicians, nurses, OT, PT, social workers, and interdisciplinary team.
• Participate in care coordination and communicate changes in patient condition.
• Maintain a professional and compassionate approach while working independently in patients' homes.
