Job summary

ConnectedHealth is seeking a Speech-Language Pathologist to deliver home-based services in Coeur d'Alene, Idaho. The role focuses on improving communication, cognition, swallowing, and overall quality of life through individualized therapy.

Responsibilities include evaluating, planning, and delivering interventions; educating patients and families; documenting care; and collaborating with an interdisciplinary team while traveling to patient homes statewide.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Active Idaho licensure or eligibility to pursue licensure.
• ASHA CCC is required per licensing standards.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide speech-language pathology services to patients in their homes.
• Evaluate patients with speech, language, cognitive, and swallowing disorders.
• Develop individualized treatment plans based on patient needs and goals.
• Deliver therapeutic interventions to improve communication, cognition, and swallowing.
• Assess and treat neurological, medical, and age-related conditions.
• Educate patients, families, and caregivers on plans and exercises.
• Recommend communication strategies and adaptive equipment when indicated.
• Monitor progress and adjust plans as needed.
• Complete accurate clinical documentation per home-health standards.
• Collaborate with physicians, nurses, therapists, and social workers.
• Participate in care coordination and communicate condition changes.
• Maintain professionalism and work independently in patients' homes.
